Classification and influencing factors of fear of progression in young and middle-aged patients with acute myocardial infarction in early rehabilitation

Objective To analyze the situation and latent profiles of fear of progression in young and middle-aged acute myocardial infarction(AMI) patients in the out-hospital early rehabilitation stage,and to explore the influencing factors of different profiles. Methods Through convenience sampling,young and middle-aged AMI patients who were hospitalized in a tertiary hospital in Guangzhou from January 2021 to August 2022 were recruited. The investigation was processed 1 month after hospital discharge. The demographic questionnaire,the Chinese version of the Fear of Progression Questionnaire-Short Form(Fop-Q-SF) and the Perceived Stress Scale(PSS),and the Social Support Rating Scale(SSRS) were used for investigation. Latent class analysis(LCA) was used to explore the latent profiles of fear of progression in young and middle-aged AMI patients. The influencing factors of latent profiles were identified by univariate analysis and multivariate Logistic regression. Results The total score of the Chinese version of the FoP-Q-SF in 352 patients is 44(28,51). The fear of progression in young and middle-aged AMI patients is divided into 3 latent classes,namely “low-risk fear”(23.86%),“high-risk fear”(20.46%),and “severe fear”(55.68%). The influencing factors include perceived stress,perceived loss of control,objective social support,availability of social support,and chronic comorbidity situation(all P<0.05). Conclusion During out-hospital early rehabilitation,the level of fear of progression in young and middle-aged AMI patients is high with heterogeneity,which could be divided into 3 latent profiles. Nurses should focus on patients with high-risk fear and severe fear and conduct early targeted prevention and mental care according to the characteristics and influencing factors of different latent profiles.

Meta-synthesis of qualitative research on the experience of elderly patients with chronic disease participating in telemedicine

Objective To systematically review and synthesize the experiences of elderly patients with chronic diseases participating in telemedicine,in order to provide references for improving the accessibility and experience of telemedicine for elderly patients. Methods We searched databases,including PubMed,Embase,Cochrane Library,Web of Science,Scopus,CINIHL,ProQuest,CNKI,WanFang data VIP and SinoMed,for the qualitative research on the attitude and experience of elderly patients with chronic diseases in using telemedicine. The retrieval time limit is from January 2000 to March 2022. We use the standardized qualitative research evaluation tool developed by the Joanna Briggs Institute to evaluate the quality of the literature. Results A total of 22 studies were included. 92 research results were summarized into 12 categories and 3 synthesized findings according to their similarities. The 3 findings are the good experience,poor experience,and the preference and expectation of telemedicine treatment in elderly patients with chronic diseases. Conclusion The adaptive experience of elderly patients with chronic diseases participating in telemedicine treatment is diversified. It is necessary to improve the telemedicine care service system from the perspectives of telemedicine devices,hospitals,communities,families and individuals,and improve the accessibility and experience of elderly patients with chronic diseases using telemedicine care services.

Objective To analyze and summarize the psychosocial adaptation experience of young and middle-aged patients after their first-time acute myocardial infarction. Methods We searched databases including PubMeds,Embase,the Cochrane Library,Web of Science,Scopus,CNKI,WanFang data,VIP and Sinomed from inception to November 2020,to collect qualitative studies on the psychosocial adaptation experience of young and middle-aged patients after their first-time acute myocardial infarction. The quality of included studies was evaluated according to JBI Critical Appraisal Tool for qualitative studies in Australia. The results were integrated by integrating methods. Results A total of 14 studies were included. 48 complete findings were grouped according to their similarities to form 4 categories. The integration results were finally obtained. The psychosocial adaptation experience of young and middle-aged AMI patients is a process of diversified performance,stage characteristics and multiple factors. Conclusion Nurses should pay attention to and understand the psychosocial adaptation of young and middle-aged patients with AMI,so that targeted interventions can be provided to help patients return to work and society better and improve their quality of life.

Sinicization,reliability and validity of the quality of life scale in women with gestational diabetes mellitus

Objective The quality of life scale for women with gestational diabetes mellitus(GDMQ) was translated into Chinese,and its cultural adjustment,reliability and validity were tested.Methods The original scale was translated by the methods of translation,synthesis,back translation and expert committee;the cultural adjustment and content validity of the scale were evaluated by the cognitive interview and expert consultation;272 women with GDM were investigated by convenience sampling method. The reliability and validity of the scale were evaluated.Results The Chinese version of GDMQ contains 5 dimensions,which are consistent with the original scale,namely concerns about high-risk pregnancy(with 10 items),perceived constraints(with 7 items),complications of GDM (with 4 items),medication and treatment(with 6 items),support(with 5 items). The S-CVI/UA was 0.81,the I-CVI was 0.86-1.00;the S-CVI/Ave was 0.97. The Cronbach’s α coefficient of the total scale was 0.914,and the Cronbach’s α coefficients of each dimension ranged from 0.656~0.920. The test-retest reliability of the total scale was 0.897.Conclusion The Chinese version of GDMQ has good reliability and validity,which can be used to evaluate the quality of life of pregnant women with GDM. It can provide corresponding bases for clinical medical staff to make health education strategies and targeted nursing measures.

Development and reliability and validity test of treatment compliance scale for children with autism spectrum disorder(parental version)

Objective To develop a treatment compliance scale for children with autism spectrum disorder(ASD)(parental version),and to evaluate the reliability and validity. Methods Based on the conceptual model of drug self-management and evaluation theory of rehabilitation prescription compliance,we formed a pretest scale by literature review,interview and expert consultation. To analyze items of the scale and test the reliability and validity,we processed a survey among 290 parents of children with ASD in outpatient department of 2 tertiary hospitals and 3 children’s rehabilitation centres in Guangzhou from August 2020 to January 2021. Participants were selected by convenience sampling. Results The treatment compliance scale for children with ASD includes 2 subscales. Subscale 1 is medication compliance scale(optional),with 4 items in total. Subscale 2 is a rehabilitation treatment compliance scale,consisting of 18 items in 3 dimensions. The cumulative variance contribution rate of subscale 1 was 72.66%. Regarding Subscale 2,there are 3 common factors being extracted,with cumulative variance contribution rate of 61.71%. Subscale 1 and subscale 2 had the same content validity index(CVI) of 0.930,and CVI of each item ranged from 0.800 to 1.000. The Cronbach’s α coefficients of subscales 1 and subscale 2 were 0.870 and 0.902,and the Cronbach’s α coefficients of each dimension were 0.760~0.909. Conclusion The treatment compliance scale for children with ASD(parental version) has good reliability and validity,and it can be used to evaluate the treatment compliance of children with ASD. This scale provides scientific support for further optimization treatment and nursing of ASD children to improve the prognosis of ASD children and reduce the burden of disease.
